ITEM NO. 1     PRESENTATION by Chief Operating Officer Sue Wozniak of OSF Saint Francis Medical Center with Request to Receive and File, and Communication from the City Manager and Assistant City Manager Requesting to Concur with the Administration's Direction to IMPLEMENT the CONTINOUS IMPROVEMENT PROGRAM of SIX SIGMA as the City's Strategy to Meet Customer's Needs in Providing Cost Effective Municipal Services.

 

ITEM NO. 2     CONSIDERATION OF CONSENT AGENDA ITEMS BY OMNIBUS VOTE, for the City of Peoria, with Recommendations as Outlined:

A.                  NOTICE OF LAWSUIT on Behalf of RAY WOZNIAK Alleging his Property Located at 2032 W. Forrest Hill Became Flooded as a Result of the Forrest Hill Improvement Project on or about the year of 2000 or 2001, with Recommendation to Receive and File and Refer to the Legal Department.

 

B.                 Communication from the City Manager and Police Chief Requesting Approval of the LOW BID of SHORE GALLERIES INCORPORATED, in the Amount of $11,141.21, for the ANNUAL PURCHASE of FACTORY AMMUNITION for Police Duty Use.

 

C.                 Communication from the City Manager and Police Chief Requesting Approval of the LOW BID of ULTRAMAX AMMUNITION for a TWO-YEAR CONTRACT, in the Amount of $26,270.00, for the ANNUAL PURCHASE of RELOAD AMMUNITION Used for Training.

 

D.                 Communication from the City Manager and Director of Public Works Requesting Approval of VENDORS for STREET MAINTENANCE MATERIALS, as Outlined.

 

E.                 Communication from the City Manager and Fire Chief Requesting Approval to Enter Into a CONTRACT with the LOW BIDDER, IWIRC, in an Amount Not to Exceed $100,000.00, for the PEORIA FIRE DEPARTMENT WELLNESS/ FITNESS INITIATIVE.

 

F.                  Communication from the City Manager and Director of Public Works Requesting an EXTENSION of the ALARM MONITORING and MAINTENANCE SERVICES CONTRACT with OBERLANDER ALARM SYSTEMS, INC. to Include $2,376.00 (Monitoring), $6,800.00 (Maintenance) and $1,876.00 (Quarterly System Inspections).

 

G.                 Communication from the City Manager and Director of Planning and Growth Management Requesting to Receive and File for Public Disclosure a POTENTIAL CONFLICT OF INTEREST Involving a HOME IMPROVEMENT PROGRAM APPLICATION so the CONFLICT Can be Submitted to HUD for an EXCEPTION DETERMINATION.

 

H.                 Communication from the City Manager and Director of Planning and Growth Management Requesting to Concur with the Recommendation from the Administration to REVISE the EMPLOYER ASSISTED HOUSING (EAH) PROGRAM, as Outlined.

 

I.                     Communication from the City Manager and Director of Planning and Growth Management Requesting to Concur with the Recommendation from the Administration to (1) AMEND the 2007 HUD ANNUAL PLAN by Adding a LEAD HAZARD CONTROL MATCH ACTIVITY, (2) AMEND the 2007 HUD ANNUAL PLAN to REFLECT the ACTUAL FUNDING ALLOCATION, and (3) AMEND the 2005-2009 CONSOLIDATED PLAN and 2007 ANNUAL PLAN RELATED to RENTAL ASSISTANCE, as Outlined.

 

J.                   Communication from the City Manager and Director of Planning and Growth Management Requesting Adoption of an ORDINANCE Amending the CITY OF PEORIA BUDGET for FISCAL YEAR 2007 Relating to the REVISED 2007 HUD ENTITLEMENT BUDGET, as Outlined.

 

K.                 Communication from the City Manager and Corporation Counsel Requesting Adoption of an ORDINANCE Amending CHAPTER 30 of the Code of the City of Peoria Pertaining to INCREASING TAXICAB OPERATOR LICENSE FEES.

 

L.                  REPORT from CITY TREASURER REGINALD A. WILLIS for JANUARY 1, 2006 through DECEMBER 31, 2006.

 

ITEM NO. 3     Communication from the City Manager and Economic Development Director Requesting Authorization to Accept the Offer from Iron Front, LLC to PURCHASE the CITY-OWNED PROPERTY Known as the EDGEWATER BUILDING at 414 S.W. WATER, in the Amount of $140,000.00, Subject to Various Provisions, and Authorization for the City Manager to Execute the Necessary Documents.
