ITEM NO. 1     Communication from the City Manager and Director of Planning and Growth Management Requesting Approval of the Following:

 

A.                  RESOLUTION Approving the ANNEXATION AGREEMENT for Property Located at 9123 N. RADNOR ROAD, Identified as Parcel Identification Number 08-36-300-015, Located Within One and One-Half Miles of the City of Peoria, with Conditions.

 

B.                 ORDINANCE Annexing Territory Contiguous to the City of Peoria - 9123 N. RADNOR ROAD.

 

ITEM NO. 2     Communication from the City Manager and Director of Planning and Growth Management Requesting Adoption of an ORDINANCE Rezoning Property from the Present Class RR (County of Peoria) District to a Class C-2 (Large Scale Commercial) District for the Property Located at 9123 N. RADNOR ROAD.

CULLINAN & SON, INC., in the Amount of $10,530.00 for GUARDRAIL REPLACEMENT for Emergency Repairs to FORREST HILL AVENUE Between Lehman Road and Molleck Drive.

 

B.                 Communication from the City Manager and Director of Human Resources Requesting Authorization for the City Manager to Enter Into a CONTRACT with CALLENDER & CO. as the LOW BIDDER for PROPERTY and CONTENTS INSURANCE, in the Amount of $58,332.00.

 

C.                 Communication from the City Manager and Director of Inspections Requesting Authorization for the City Manager to Enter Into a CONTRACT with RIVER CITY DEMOLITION, in the Amount of $36,991.00 for the Court Ordered Demolition of the COMMERCIAL STRUCTURE Located at 1117 S.W. ADAMS.

 

D.                 Communication from the City Manager and Finance Director/Comptroller Requesting Approval to REPROGRAM $50,000.00 of CAPITAL FUNDS from the Community Initiative Project to OPERATION CEASE FIRE.

 

E.                 Communication from the City Manager and Director of Public Works Requesting Adoption of an ORDINANCE Amending Ordinance No. 14,801 MODIFYING the BOUNDARIES of a RESIDENTIAL PARKING PERMIT PROGRAM AREA Enacted November 18, 1997, Removing the West Side of MAPLEWOOD AVENUE Between W. Main Street and Bradley Avenue.

 

F.                  Communication from the City Manager and Director of Planning and Growth Management Requesting Adoption of an ORDINANCE Amending Ordinance No. 13,307, an EXISTING SPECIAL USE in a Class R-1 (Single-Family Residential) District, and a Class R-2 (Single-Family Residential) District for ELDERLY HOUSING to Add a PHYSICAL THERAPY ADDITION for the Property Located at 5600 GLEN ELM DRIVE, with Conditions.

 

G.                 Communication from the City Manager and Corporation Counsel Requesting Approval of the SITE APPLICATION for a CLASS B (Restaurant) LIQUOR LICENSE for 4630 N. UNIVERSITY, UNITS 51 & 52, with Recommendation from the Liquor Commission to APPROVE.

ITEM NO. 7           Communication from the City Manager and Corporation Counsel Requesting to Concur with the Recommendation from the Liquor Commission to DENY the SITE APPLICATION for a CLASS C-1 (Package Goods) LIQUOR LICENSE for 3405 S.W. ADAMS.

(06-467)          Communication from the City Manager and Director of Human Resources with Recommendation to Concur with the Joint/Labor Management Health Care Committee Recommendation to Approve the 2007 CITY OF PEORIA HEALTH CARE PLAN CHANGES and PREMIUM CONTRIBUTION RATE CHANGES for ACTIVE EMPLOYEES Including Maintaining the Two 10% Subsidies for Retirees Until 1/1/2010, at Which Time the Subsidies will be Reviewed, and Subject to the Contribution Rates on Other than Individual Coverage Increasing to 85%/15% on 1/1/2007, Remaining at 85%/15% on 1/1/2008, if the $2.35 Million Reduction in the Annual Required Contribution (ARC) is Realized in 2007, Increasing to 80%/20% on 1/1/2008, if the $2.35 Million Reduction in the ARC is Not Realized, and Increasing to 80%20% on 1/1/2009, with Plan Design Changes as Set Forth.
